New patients are welcome to register with any of our branch surgeries.

To register, please ask the receptionist for a new patient registration form which you will need to fill before being registerd with the practice. You can also download the form from this website.

You will be asked for copies of documents for identification such as passport or drivers license.

Soon after registration, you will be asked to attend the surgery  for a new patient check with the nurse or health care assistant. This is very important and this will be necessary to continue to be registered with the practice.

Your records from your previous GP will automatically be transferred to us and relevant records uploaded to our computerised database. This may take up to a month to complete so it would be useful to have details of the current medications you take at hand.

Patients temporarily moving into our area can be registered as temporary patients for up to 3 months. A separate form is available for temporarily registering patients.
