Attached Staff

Midwives 

Community Midwives are attached to the practice. They are concerned with the care of all pregnant mothers and visit mother and baby at home in the early days after the delivery. They may also visit pregnant mothers at their homes in special circumstances. In addition, they are also present for antenatal services at:

District Nursing Staff 


A team of District Nurses is attached to the practice. They are based at their site at Clayton Le Moors and cover patients registerd at all our surgeries. 

They make visits to patent's homes. Their role is one of health promotion, giving advice and promoting self-care. They offer support and deliver care for people who are poorly (and their relatives), involving other agencies as appropriate. They also undertake tasks such as dressing wounds, blood pressure monitoring, blood tests, administering medications for patients who are housebound and cant make it to the surgery.


They have a special role in care of terminally ill patients and do provide extra support for patients at this time. Counselling is available as part of their role for the terminally ill or bereaved.

Community Matron

The Community Matrons role is to keep under review patients in the community with complex health issues. With their input poorly patients in the community can be more closely monitored and any deterioration in their health can be quickly picked up and measures put in place for more active management. They are highly trained to examine,  assess and if necessary prescribe required medications for patients under their care.

They liase closely with the surgery and the GP is kept aware of ill patients under their care.

With their input, it may be possible to keep patients stable at home avoiding un-necessary hospital admissions

Patients can be  referred to the community matron by the GP or other community based agencies and sometimes following discharge from the hospital.

 

Health Visitors


The Health Visitors are involved in promoting the health of all patients and offering advice and support, particularly to expectant mothers, children from birth to five years and those with disabilities or problems.


The Health Visiting team is based at their own site in  Oswaldtwistle and can be contacted at their Oswaldtwistle clinic on 01254 355596/7/8/9 (answerphone 355595) from Monday to Friday between 9.00am and 5.00pm for advice or to arrange a home visit.


Health visitors are on site at the surgeries during baby clinics and will be happy to offer any advice and support at the clinics. Please see the receptionist at the surgery if you would like to see the health visitor during the Baby clinic.
